In Pennsylvania, there are several Home and Community-Based Services (HCBS) Waiver Programs available to provide support for individuals who wish to remain in their home and community rather than enter a nursing facility.
DEC Staff can assist in enrollment in waiver programs, as well as support service coordination for consumers already enrolled in a waiver program.
Our Service Coordinator, Shannon Orehek, annually evaluates the Consumer’s strengths, needs, preferences and desired outcomes. The Service Coordinator designs a service plan to meet the Consumer’s goals which may include personal assistance service, durable medical equipment, home modifications and skilled services. The Service Coordinator monitors the goals throughout the year and assists the Consumer with problem solving and provider referrals.
DEC serves Independent, Attendant Care and ACT 150, Obra, Commcare, and Aging waivers for Lancaster, Lebanon, York and Dauphin counties.
